Transitional Care (Swing Bed) Model in Critical Access Hospitals: Underutilized Opportunity for Acute Hospitals and Key Stakeholders

This presentation on Transitional Care (swing bed) model in critical access hospitals will highlight the benefits to Minnesota acute care hospitals by promoting shorter length of stay, lower readmission rate, and higher likelihood of home discharge. The swing bed program has significant advantages relative to other post-acute care options. Transitional Care (swing bed) provides access to high nurse staffing, culture of safety, hospital level services including on-site lab, radiology and ability for a physician to address an acute change of condition. Minnesota and national experience with the model will be shared including the results of an ongoing Minnesota Department of Health funded Flex grant that involves 8 critical access hospitals. Essential elements of implementing a successful program and strategies to overcome common barriers to maximize program success will be discussed. Creating meaningful connections for critical access hospital swing bed teams to meet with key acute care hospital stakeholders to share the value proposition is essential for optimal success. Transitional Care (swing bed) is a markedly underutilized high quality post-acute care pathway that has a very strong business case for Minnesota critical access and acute care hospitals and has the potential to improve long term rural hospital viability.

Objectives

1.      Promote the necessary on going collaboration for critical access hospitals with swing bed programs to connect and communicate the value equation of their swing bed program to partner acute care hospitals.

2.      Be able to describe at least 3 unique quality attributes of swing bed care that is advantageous relative to other post-acute care options. 

3.      Be able to highlight the business case in implementing this model for both acute and critical access hospitals.

4.      Provide three activities acute care hospitals can implement to drive growth of swing bed care in Minnesota rural communities, reduce excessive hospital lengths of stay and improve their hospital flow.
