The health care landscape is becoming more connected and diverse. Health care leaders may oversee a myriad of facility types, including skilled nursing facilities (SNF) and long-term care (LTC) facilities. The patient-driven payment model (PDPM) represents a shift in thinking, documentation and reimbursement for SNFs.
