Regional patient and family engagement (PFE) and readmissions workshops will be offered in four locations to support Minnesota hospitals and health systems in their efforts to reduce readmissions and to provide coordinated care at transitions. Workshops will take a patient-centered approach using curriculum developed with input from many patient and family advisors.The workshops will be facilitated by MHA quality and patient safety division team members.
