Every hospital, including critical access hospitals (CAHs), needs to attend this important web conferencethat will cover the new changes to the Centers for Medicare and Medicaid (CMS) discharge planning standards that became effective Nov. 29, 2019. These apply to all hospitals that accept Medicare and Medicaid, and for the first time will apply to CAHs. CMS scaled back on many of the proposed rules about which hospitals had expressed concern. 

This web conference will review regulations on timely access to medical records for patients, the discharge planning process, discharge instructions, discharge planning requirements, transfers to other facilities, assessment of readmission within 30 days, caregiver rights and recommendations, reduction of factors that lead to preventable readmissions, timely discharge planning and more. 

This program will also cover sections that were not adopted by CMS, but were included in recommendations, including the prescription drug monitoring program, the 24-hour requirement to initiate a discharge plan, eight items to be in the discharge planning assessment, 21 items to be included in the transfer form, medication reconciliation, the discharge summary and more.
